Client Acknowledgement
• Nick's Mobility Plus is a Non-Emergency Transportation Provider servicing Harford County.
• We provide transportation for seniors or to anyone who requires wheelchair or ambulatory transportation services.
• All trips provided by Nick's Mobility Plus are recorded by dash cam that records the front of the vehicle. The inside of the vehicle captures audio and video recording.
• Nick's Mobility Plus LLC vehicles are ADA compliant.
• If you believe your wheelchair is larger than a standard size wheelchair or if you have a motorized wheelchair please call (571) 464-9794 to confirm transportation accessibility.
• All trips are subject to scheduling and availability.
• Trips must be scheduled 48 hours in advance. (Last-minute trips will be accepted based on availability)
• Trips can be scheduled by phone Monday- Friday 7:00 am - 4:00 pm or through our website at www.nicksmobilityplusllc.com
• A deposit is required for all trips to be booked.
• Additional business hours are offered to Members and to clients who need transportation to BWI Monday - Friday 7:00 pm- 8:00 pm
• Out of County trips are limited to 25 miles out of Harford County. Please Call for additional information.
• Full-day and Half day trips are available to clients who want a driver to stay with them for the duration of their appointment 4 or 8 hours. Unlimited trips during the scheduled time.
• Clients can be accompanied by (1) passenger at no additional cost.
• Nick's Mobility Plus LLC doesn't provide transportation out of the state of Maryland.
• If you are experiencing a medical emergency prior to your scheduled trip, please call 911.
• If you are experiencing cold or flu-like symptoms before your scheduled trip, please reschedule your ride.
• If the driver feels that the client or the client's quest is unsafe to be driven, the trip will be rescheduled.
• If a client requires assistance with safety and decision-making, they need to be accompanied by their caregiver. (No extra cost for the additional (1) passenger).
• Nick's Mobility Plus LLC doesn't transport clients under 18 years old without adult supervision.
• All clients transported by Nick's Mobility Plus LLC must be able to properly use the vehicle's safety seat belt. No child car seat or booster seat.
• If the client is ambulatory but is uncomfortable walking. A wheelchair can be provided from door to van transport (Please let the staff member know during the initial call)
• Currently staff members of Nick's Mobility Plus will not be taking clients down any steps. The client must be able to meet the driver at the lowest level of the pick-up location with no steps.
• Clients must know how to operate their equipment including motorized wheelchairs.
• If the clients wheelchair is damaged prior to the transport and can't be safely secured, the trip will be rescheduled.
• No client will be transported during snow or icy weather conditions. However some services will still be available. If it begins to snow after the client has been taken to their location, we will still provide transportation back to your destination as scheduled. (Only when safe to provide the transport).
• Driver wait service is available at a additional cost. Wait service is included in the Full Day and Half Day services.
• Clients must complete an emergency contact form prior to any trip.
• A trip can be provided for a surgical procedure; however, a trip cannot be provided for a surgical procedure in which anesthesia (other than a local) is administered.
• Clients must pay for their items picked up by staff members. Please call for additional information.
• In the event of an emergency the driver is instructed to pull the vehicle over to a safe location and call 911.
• The driver will follow the instructions of the 911 operator/dispatcher until emergency responders arrive.
• NO smoking in any of the vehicle used by Nick's Mobility Plus LLC
• All drivers are certified in CPR/BLS, NEMTAC, Q'STRAINT, AND CTAA (Pass Trained)
• Please plan to be ready 15 minutes prior to your scheduled pick up time.
• If your driver has not arrived within 15 minutes after your schedule pick up time, please call (571) 464-9794
